Where level access is the goal, the fall has to be formed within the structure rather than added on top.

The short answer

Every layer added under the tile has to be paid for at the doorway. Build-up planning avoids trip steps and doors that no longer close.

The detail that matters

    Backer board, decoupling matting, adhesive and tile can easily add 20-30mm; the door undercut and threshold detail must be planned for that.

Only if the build-up is planned. Doors are often trimmed or rehung as part of the job.

What threshold works between tile and carpet?

A flat trim set to the finished heights, fitted after both floor finishes are down.
